Heroic Care Ltd - Transforming Lives

Supported Living

Support to live in your own home, with choices about your daily life and the help you receive.

How Does Supported Living Work?

We make decisions with you about your home and support. Following assessments by Adult Social Care and our team, we explore whether living alone or sharing a home is right for you. Your family, friends and the people you choose can be involved.

Your assessment guides the hours of support and staffing you need. You may share staff support with others, have one-to-one support, or need support from two or three staff.

Getting To Know You

We take time to understand your needs, your interests and what matters to you. Your care folder brings together the information needed to support you.

It includes your care plan, health action plan and communication passport. We also include any positive behaviour support plans and risk assessments, alongside your life experiences, likes, dislikes and circle of support.

We plan activities and learning with you, including ways to stay active. These are shaped around your interests and goals, helping you build skills and confidence at your own pace.

Finding The Right Home And Support

You can be involved in choosing the staff who support you. We train our team to understand your needs and the support that works for you.

We can help you find a suitable home. When you move in, you become a tenant.

If you need help with benefits or finances, our manager Josh Henry can support you. This includes help with housing benefit and other benefits you may be entitled to.

Promoting Your Independence

We can help you keep your home clean and tidy, report maintenance issues, shop for food and prepare nutritious meals.

You may need guidance, supervision or more hands-on help. We provide the support you need while encouraging you to do what you can for yourself.

We respect your dignity, rights, choices and identity. Your support is shaped around you.

We work with you towards your goals, helping you build independence and enjoy a fulfilling life.
